New patient? Please print off the New Patient Welcome Forms and bring them with you to your first appointment. This will allow more time spent face to face for your initial exam.

It is important that full intake and history be completed so we can get to know you better and provide you with an exceptional level of treatment based on your needs. A physical exam will be done based on your level of need and then we will present you with all your options before beginning treatment. You may expect your first visit to take anywhere from 45 min to an hour and a half.
